What is the purpose of this form?
The purpose of the minor talent release form is get
permission from a parent/guardian for someone who is under 18 years of age to
take part in filming.
Why would it be useful in creative industries?
The form would be useful in the creative industries if a
minor is needed during filming and it requires parental permission for that
minor to take part in filming.
Recce form
What is the purpose of this form?
The purpose of this form is to assess a location that may be suitable for the purpose of filming, available at the time that it is needed and is suitable distance from home to the location. It can be also used as a storage for location information.
Why would it be useful in creative industries?
It would be used in the creative industries to keep track of the locations that have been used during the process of scouting the locations for use to do filming of a video or taking image for the industry.
Adult talent release
What is the purpose of this form?
The purpose of the adult talent release form is to get
permission from the person who is at legal age to use them during the filming
process.
Why would it be useful in creative industries?
The form would be useful in the creative industries if any
person aged 18 and over wishes to take part in filming, they would then sign
this form, giving permission to the person in charge of the filming.
Location release
What is the purpose of this form?
The purpose of a location release form is an agreement
between the owner of the location where filming will take place and the
crew member who would be in charge of the filming.
Why would it be useful in creative industries?
It would be useful in the creative industries if a location
has been selected for filming, permission is needed from the owner of the
location to be used, and the crew are responsible for any damages to the
location.
Risk assessment
What is the purpose of this form?
The purpose of the risk assessment form is to inspect the
location for any hazards around it and any hazards found will be removed from
the location safely.
Why would it be useful in creative industries?
The form would be useful in creative industries as the
location may have hazards that may need identifying and possibly removing those
hazards.
Why/when, would I need to use these documents?
I may need to use these documents when thinking about the
location where I will do the filming, obtaining the permission from the owner
of that location to film, obtaining permission for any adults or minors taking
part in filming and look for any risks in the chosen location.
